Nursing Notes
Report received from day nurse at 1915. Patient observed laying asleep in bed. Patient easily aroused with verbal stimuli, alert and oriented x4, pleasant. Denies pain but c/o being hot and sweaty, temperature adjusted and lighting dimmed to promote sleep. Respirations even, unlabored and patient in no apparent distress at this time. Bed is low, wheels are locked, and call light is within reach.
Bed Alarm Activated: Bed alarm activated at this time, patient was found to attempting to ambulate to bathroom unassisted. Patient assisted to bathroom and then back to bed. Patient educated on fall precautions and the need to call for assistance before attempting to ambulate. Patient verbalized understanding. Bed low, call light in reach, belongings at bedside. Bed alarm re-armed at this time.
